FRAUD REFERRAL FORM
You can report suspected cases of fraud in the following program areas: Temporary Assistance (Welfare), SNAP (Food Stamps), Medicaid, HEAP, and Day Care. Additionally you may report any individual who is a day care provider or landlord, and is receiving payments from the Department of Social Services-Economic Security for services not being provided. You can also report stores who exchange Food Stamps for cash, individuals who sell their Food Stamp Benefits for cash, or who purchase food with their benefits, and sell the food to other retailers. Examples of common fraud situations involve recipients who fail to report receiving income or fail to report everyone residing in the home.
